The most effective way to report an incident is via phone. You may reach as at 408-837-0116. We have our 24/7 Support Team available to assist you, 365 days a year, even on weekends. You will be expected to fill out an Incident Report Form Questionnaire and then once we have received the other parties Incident Report as well, you may expect a timely follow-up.
